Be on the Lookout for Medical and HSA Tax Forms

The following tax forms were sent to you in late January:

• Form 1095-C documents that you were covered by a CMSU medical plan.
• Form 1099-SA shows the amount of money you spent from your HSA during the tax year.
• Form 5498-SA shows the amount of money deposited into your HSA for the tax year by CMSU and you.
• Form 8889 is the form you fill out and submit with your tax return.
