Long Term Care Benefit Summary

You have two Long Term Care benefit plans to choose from:
- Low Option: $3,000 per month for nursing facility coverage or $50 daily up to $1,500 per month for professionalhome care coverage.
- High Option: $6,000 per month for nursing facility coverage or $100 daily up to $3,000 per month for professional home care coverage.
You also have several different "buy up" options. You can choose the option or combination of options that best suits your needs, subject to the evidence of insurability requirements described under Enrollment.

The plan pays benefits after you satisfy the elimination period. You will only need to satisfy the elimination period once in your lifetime.
There is a lifetime individual maximum for all long term care benefits. The Low Option lifetime maximum is $108,000 and the High Option lifetime maximum is $432,000. If you elect inflation protection as a "buy-up" option, this maximum will increase from year to year to help keep up with inflation, as provided under that option.
