Filing Claims
You do not need to file a claim if you go to a VSP Network provider. When you call the VSP provider and schedule your appointment your provider handles all the paperwork for you.
If you go to a non-VSP doctor, you will have to pay for services and then submit your itemized receipts and any other relevant information to VSP for reimbursement. Relevant information includes:
- The name, address, and phone number of your non-VSP provider
- Your name, Social Security number, phone number, address, and employer's name
- The patient's name, date of birth, phone number, address, and relationship to you
You will be reimbursed for costs up to the plan's benefit limits. You have six months to submit such claims to:

If your claim is denied, or partially denied, you have the right to appeal the decision. See Filing Claims under Legal Information for details about appealing your claim.
If you are an enrollee in the Plan and have a complaint about VSP service or a claim payment, you can contact VSP by using the form which is available through the VSP Customer Service Department. You can call Customer Service at 1-800-877-7195, Monday through Friday, from 6:00 a.m. to 6:00 p.m., Pacific Time. You can also file your complaint by writing VSP at:

If you have an emergency grievance, or one that you feel has not been satisfactorily addressed by VSP, you should contact the California Department of Corporations, which is responsible for regulating health care service plans. The department has a toll-free telephone number, 1-800-400-0815, for receiving complaints about health care plans.
