Eligibility and Enrollment
You are eligible to participate in the Long Term Disability Plan if you are scheduled to work at the Company a minimum of 20 hours per week in active employment. Active employment means that you are actively at work for Canon Medical Systems:
- On a full-time basis and paid regular earnings.
- For at least 20 hours per week at the Company's usual place of business or at a location to which the Company's business requires you to travel.
Employees who are temporary, seasonal, or who are not legal residents working in the United States are not eligible.
Individuals whose employment status is being continued under a severance or termination agreement are not considered in active employment.
Enrollment is automatic. You are not required to make contributions to the plan. The Company pays the full cost of your coverage. Keep in mind that any benefit payments you receive from this plan will be treated as taxable income.
Coverage begins on your date of hire or the first day you are an eligible employee. If you are not in active employment on the day your coverage under this plan would normally become effective, coverage will be effective the day after you return to active employment status for one full day.
Your participation in the plan will stop on the earliest of the following dates:
- The date the plan is canceled
- The date you are no longer a member of the covered classes of employees or your covered class is no longer covered
- The last day you are in active employment
Changes in Employment Status
Changes in employment status can affect your coverage:
- If your hours are reduced to less than 20 hours per week, for reasons other than disability, you will be covered to the end of the month following the month in which your reduced hours begin.
- If you are on an unpaid personal leave of absence, you will be covered to the end of 30 working days from the date that your unpaid personal leave begins.
- If you are on an approved paid leave of absence, you will be covered for up to one year from the date that your leave begins.
- If you are on an approved family or medical leave (FMLA), you will be covered for up to 12 weeks following the date coverage would have terminated.
