How the Plan Works
The following information describes beneficiary designations, assigning your insurance, how to file a claim, and how benefits are paid.
It is essential that you choose a beneficiary or beneficiaries for your insurance and keep the information updated. Your beneficiary designation is a legally binding document. It ensures that benefits are paid to the correct person in the event of your death.
You may name anyone as your beneficiary and you can change your beneficiary at any time. As your personal life changes (e.g., marriage, divorce, birth, adoption, death of a beneficiary), be sure to review your beneficiary designation and change it, if needed.
If you suffer a covered loss other than death, you are the beneficiary for optional AD&D insurance. For dependent AD&D insurance, you are automatically the beneficiary in the event of a covered death or loss.
Claims for covered losses of life or limbs should be filed as soon as possible following the loss. Contact your Benefits Department for assistance with completing the necessary claim forms.
Optional AD&D insurance provides benefits in the event of death or a covered loss that occurs within 365 days of an accident.
Benefits are normally paid in a single lump sum to you if you suffer a covered loss, or to the named beneficiary in the event of your death.
If no beneficiary designation is in effect, benefits will be paid in the following order:
- Surviving spouse or domestic partner
- Surviving children in equal shares
- Surviving parents in equal shares
- Your estate
The person receiving benefits must be living on the tenth day after your death.
In the event of an overpayment, the insurance company will be entitled to a refund of the excess amount.
