Who's Eligible
PLEASE NOTE: UNUM CANCELLED THE GROUP LONG TERM CARE PLAN ON MARCH 31, 2018
All regular full-time employees of Canon Medical Systems who are regularly scheduled to work 20 hours or more per week are eligible to enroll in the Long Term Care Plan.
Your eligible dependents may also enroll, including certain
extended family members. For purposes of this plan, dependents must
be under age 85 to be eligible and are defined as your:
- Legally married spouse
- Domestic partner—a "domestic partner" is someone to whom you are not married, is at least 18 years of age, mentally competent, not married to anyone else, and is not so closely related by blood that a legal marriage would be prohibited. To be eligible for benefits, "domestic partners" who are registered with any state or local government registry, or who have completed a Partner Enrollment Guide that confirms these facts and certifies that you and your domestic partner have met all of the following criteria:
- Be in an intimate, committed relationship of mutual caring
- Share the same principal residence
- Agree to be responsible for each other's basic living expenses during the partnership and agree that anyone who is owed expenses can collect from either partner
- Neither partner has had another domestic partner in the last six months (this condition does not apply if you had a partner who recently died)
- Children and step children of you or your spouse/domestic partner who are age 18 or over
- Parents, step parents, parents-in-law, and step parents-in-law of you or your spouse/domestic partner
- Grandparents, step grandparents, grandparents-in-law, and step grandparents-in-law of you or your spouse/domestic partner
- Adult siblings, adult step siblings, adult siblings-in-law, and adult step siblings-in-law of you or your spouse/domestic partner
