Life/AD&D Insurance Summary

Basic life insurance is company-paid and provided to all eligible employees.

In addition to company-paid basic life insurance, you have the opportunity to buy supplemental life/AD&D insurance at competitive group rates. If you elect coverage for yourself, you may also purchase dependent life insurance coverage for your spouse/domestic partner and dependent children. Proof of good health may be required for certain supplemental employee and dependent life insurance amounts as described below.

Field Sales Personnel

Annual base salary for field employees means your annual salary or wage.

  

Basic Company-Paid Life Insurance for Employees

The amount of your basic Company-paid life insurance depends on your years of service:

  • Field sales personnel with less than one year of service: $100,000
  • Field sales personnel with one year or more of service: $100,000 (your basic insurance will be adjusted each January 1 to reflect 2x the prior calendar year's W-2 earnings, not to exceed $600,000

Proof of good health is not required for employee basic life insurance.

  

Supplemental Employee-Paid Life/AD&D Insurance for Employees

You may purchase supplemental life/AD&D insurance for yourself in the amounts of $100,000, $200,000, $300,000, $400,000, $500,000, or $600,000. 

You may elect coverage when you are first eligible. Proof of good health (also referred to as Evidence of Insurability) is not required if you request coverage within 31 days of becoming eligible. 

You may also elect coverage during the annual Open Enrollment period:

  • If you have existing supplemental life/AD&D coverage, you may elect to increase it by one level without proof of good health (for example, $100,000 to $200,000).
  • If you do not have existing supplemental life/AD&D coverage, you may request initial coverage, but proof of good health will be required.

You will need to provide proof of good health if you apply for any amount of coverage (initial coverage or an increase) at any other time.

 

Dependent Employee-Paid Life Insurance

Dependent life insurance is available to your spouse/domestic partner and children as follows:

  • Spouse/Domestic Partner: $10,000, $20,000, $30,000, $40,000, $50,000, $75,000, or $100,000; cannot exceed 50% of employee's basic and supplemental life insurance (note: proof of good health is required as described below)
  • Child(ren): $5,000

Proof of good health (also referred to as Evidence of Insurability) is not required for the following:

  • Initial enrollment within 30 days of becoming eligible—you may request up to $50,000 for your spouse/domestic partner (you will automatically receive $5,000 child coverage). 
  • Open Enrollment—you may request an increase to existing dependent life coverage of one level, not to exceed the $100,000 benefit level for your spouse/domestic partner (you will automatically receive $5,000 child coverage). If you do not have any dependent life coverage, you may request initial coverage, but proof of good health will be required. 

You will need to provide proof of good health if you apply for any amount of coverage (initial coverage or an increase) at any other time. 

 

Age reductions

Beginning on your 70th birthday, if you are still actively employed at the Company, your basic and supplemental insurance will be reduced as follows:

  • From your 70th birthday to age 75, your life insurance will be reduced to 65%. For example, if you have $100,000 of life insurance before you turn age 70, your insurance will reduce to $65,000 on your 70th birthday.
  • From your 75th birthday and after, your life insurance will be reduced to 50%. For example, if you have $100,000 of life insurance before you turn age 70, your insurance will reduce to $50,000 on your 75th birthday.

These age reductions also apply to your spouse/domestic partner dependent life insurance when he/she reaches age 70 and 75, respectively.

  

Non-Field Sales Personnel

Annual base salary for non-field employees means your annual salary or wage, and does not include bonuses, overtime pay, and any other special compensation.

  

Basic Company-Paid Life Insurance for Employees

Your basic Company-paid life insurance is 2x your annual base salary, rounded up to next higher $1,000, not to exceed $600,000. Proof of good health is not required for basic life insurance.

  

Supplemental Employee-Paid Life/AD&D Insurance for Employees

You may purchase supplemental life/AD&D insurance for yourself in the amounts of 1x, 2x, 3x, 4x, or 5x annual base salary, rounded up to next $1,000 increment, not to exceed $600,000.

You may elect coverage when you are first eligible. Proof of good health (also referred to as Evidence of Insurability) is not required if you request coverage within 31 days of becoming eligible.

You may also elect coverage during the annual Open Enrollment period:

  • If you have existing supplemental life/AD&D coverage, you may elect to increase it by one level without proof of good health (for example, one to two times annual base salary).
  • If you do not have existing supplemental life/AD&D coverage, you may request initial coverage, but proof of good health will be required.

You will need to provide proof of good health if you apply for any amount of coverage (initial coverage or an increase) at any other time.     

 

Dependent Employee-Paid Life Insurance
  • Spouse/Domestic Partner: $10,000, $20,000, $30,000, $40,000, $50,000, $75,000, or $100,000; cannot exceed 50% of employee's basic and supplemental life insurance (note: proof of good health is required for spouse/domestic partner amounts over $50,000)
  • Child(ren): $5,000

Age reductions

Beginning on your 70th birthday, if you are still actively employed at the Company, your basic and supplemental insurance will be reduced as follows:

  • From your 70th birthday to age 75, your life insurance will be reduced to 65%. For example, if you have $100,000 of life insurance before you turn age 70, your insurance will reduce to $65,000 on your 70th birthday.
  • From your 75th birthday and after, your life insurance will be reduced to 50%. For example, if you have $100,000 of life insurance before you turn age 70, your insurance will reduce to $50,000 on your 75th birthday.

These age reductions also apply to your spouse/domestic partner dependent life insurance when he/she reaches age 70 and 75, respectively.
