Your dental plan provides essential dental care benefits to support long-term dental health.
This page provides an overview of dental coverage provided through Delta Dental. The plan gives you and your family the freedom to go to any dentist for dental care. Using network dentists saves money because they have agreed to lower, contracted rates, which means your bill will be as low as possible right from the start. Non-network dentists may charge fees that exceed the plan's usual, customary, and reasonable (UCR) guidelines, which is the plan's allowed amount for a covered service. You are responsible for any charges that exceed the UCR limit.
Delta has two networks available. The DPO/PPO network offers the deepest discounts, which translate into lower out-of-pocket costs for you. The DeltaPremier network is also available—these dentists don't discount their fees, but they have agreed to not charge more than Delta's allowed amounts. You may use network dentists from either network.
Dental benefits include the following:
- You pay an annual deductible before the plan pays most benefits:
- In-network: $50 per person and $150 per family
- Out of network: $100 per person and $300 per family
- Plan pays 100% for preventive and diagnostic services—the dental benefits deductible is waived for these services
- Plan pays 80% after the deductible for basic and restorative services
- Plan pays 50% after the deductible for major services
- Plan pays 50% for orthodontic services up to $2,000 per patient per lifetime—the dental benefits deductible is waived for these services
- The maximum amount the plan pays in any calendar year is $2,000—preventive, diagnostic, and orthodontic care do not count toward your annual dental benefits maximum, so your benefit goes farther
- No claim forms are needed when you use network dentists

Dental ID cards will be mailed to your home within 10 to 14 business days of your new hire enrollment.
