Important note: This voluntary plan was frozen effective December 31, 2020. As a result, it is no longer accepting new enrollments. If you were enrolled in the plan on December 31, 2020, your coverage will continue through payroll deduction until you cancel your coverage. Basic Company-paid critical illness insurance coverage is provided by the Voya critical illness plan.
The cost of a critical illness can be significant. Medical coverage typically covers a large part of the cost, but not all. If you are diagnosed with a critical illness, this plan will pay you a lump sum benefit—separate from and in addition to your medical coverage. The plan also includes a wellness benefit that pays you money for receiving covered health screenings.
You are eligible if you are covered by a Company medical plan or other group medical plan.
- You may purchase up to $45,000 for yourself. If you purchase voluntary coverage for yourself, you may also purchase up to $30,000 for your spouse/domestic partner. Voluntary coverage is paid on an after-tax basis through convenient payroll deductions.
- Eligible children are covered at 25% of your amount.
- Medical conditions covered by the plan include heart attack, stroke, major organ failure, permanent paralysis, end stage renal (kidney) failure, cancer, coma as the result of a severe traumatic brain injury, benign brain tumor, blindness, and occupational HIV. Partial benefits are paid for coronary artery bypass surgery and carcinoma in situ.
- Children are also covered for these additional specific childhood conditions: cerebral palsy, cleft lip or palate, cystic fibrosis, down syndrome, and spina bifida.
- Use the money any way you choose—pay your medical plan deductible or coinsurance, cover your mortgage, or take a vacation.
- You don't have to be disabled or terminally ill to receive the benefit.
- No benefits are paid within the first 30 days of coverage, unless for an injury or a listed childhood covered benefit.
- Benefit amount. The plan pays $200 for receiving a mammogram based on the recommended age guidelines below.
- Who's eligible. Employees and spouses/domestic partners who are enrolled for voluntary coverage.
- Waiting period to receive benefit. 30 days. For example, if you are eligible on January 1, schedule your mammogram to be completed on or after January 31.
- How often. The benefit is payable as follows:
- Once between the ages of 35 to 39
- Once every two years between the ages of 40 to 49, or more frequently based on the physician's recommendation
- Once every year at age 50 and older
