Health360 Rewards
Good health is a reward in itself, but employees and their spouses or domestic partners also earn incentives for participating in the Health360 wellness program. It doesn’t matter if you’re already healthy or you’re living with a health condition—you can earn rewards just by participating!
Please note that rewards for participating in the Health360 wellness program are available to all employees, regardless of health status. All of your Health360 rewards can be tracked on the “Rewards” page at your Personify Health member account.
Take steps today to earn a wellness incentive in 2026! The annual wellness incentive provides employees and their spouses/domestic partners with an opportunity to each earn $500 in wellness incentive dollars for taking an active role in their health, up to a combined total of $1,000. These wellness dollars can pay for eligible expenses in your medical plan normally paid for out of your own pocket. Earned wellness dollars are deposited into your Health Savings Account (HRA) or Health Reimbursement Account (HRA) by the end of January 2026.
You are eligible for the Annual Wellness Incentive if you are enrolled in a Company CDHP medical plan in the year you complete the incentive and the following year when wellness dollars are funded (EPO medical plan participants are not eligible for the incentive because that plan does not come with a Health Savings Account in which to deposit earned wellness dollars).
The Annual Wellness Incentive has three steps that must be completed between January 1 – November 15, 2025:
- Health Screenings. Health screenings involve you, your doctor, and a lab testing facility. They can identify if you’re at risk for a health condition before it becomes serious and impacts your quality of life.
- Health Questionnaire. The Health Questionnaire (HQ) is an online questionnaire that gives you insight into your overall health and lifestyle.
- Next Steps Consult. The Next Steps Consult is a 15-minute call to review the results of your health screenings and the Health Questionnaire.

To complete the Annual Wellness Incentive, follow these steps:
- Log in to your Personify Health account. If you haven’t created an account yet, visit the Personify Health sign-up page and follow the instructions. Spouses/domestic partners who are creating their account should use your employee number and the letter “S” at the end (e.g., 12345S).
- After you have logged in, go to the “Rewards” page and scroll down to “Annual Wellness Incentive” section. There, you’ll find links to each of the three steps and instructions to complete.
About health screenings
- If your spouse or domestic partner wishes to participate, you must each log in and download the form (forms are specific to each person).
- Because some of the screenings must be done at your doctor’s office and your doctor is the authorized person to fill out the Physician Screening Form, it may be convenient and cost-effective to combine this visit with your annual preventive exam or another regularly scheduled appointment.
- You have the option to upload the completed form yourself to your Personify Health account (fastest method) or let your doctor fax the form to Personify Health. To upload the form, take a picture of the completed and signed form at your doctor visit. Then, upload the image to your Personify Health account on the same page you downloaded the form.
Funding of wellness dollars
CMSU will fund earned wellness incentive dollars into your HSA or HRA by the end of January 2026. Because your health account is tied to your medical plan, you must be enrolled in a Company medical plan in 2025 and at the time that the money is funded.
Confidentiality
The purpose of the Annual Wellness Incentive is for you to gain valuable information about your health status. Your health information is never reported to Canon Medical Systems. The Company is only informed if you complete the Annual Wellness Incentive so that we may fund your health account.
Some information that is gathered during the wellness incentive activities is shared with our health and wellness program partners to provide rewards for healthy screening results, to ensure the accuracy of the health questionnaire, and to offer appropriate services available through our health and wellness program. Our wellness partners are required to comply with federal health and privacy laws, and do not share your personal health information with Canon Medical Systems or with any other company who is not part of our benefits program.

The annual Healthy You Challenge is designed to motivate you all year long to incorporate healthy actions into your life. You will earn points for a variety of activities described on the Personify Health “Rewards” page, and those points will turn into Rewards Cash that you can spend on a gift card.
There are four levels of rewards:
- 10,000 points = $75
- 30,000 points = $100
- 45,000 points = $150
- 70,000 points = $175
If you reach all four levels, you can earn up to $500 in Rewards Cash!

Tax note: Cash or cash equivalent prizes are considered taxable income according to the IRS. If you earn a reward, the additional earnings will be reported to the IRS, and your withholding taxes will be calculated based on the increased taxable income. The value of the prize will be shown on your paystub in the earnings section as “Prize”, and a corresponding deduction will also appear on your paystub so that you can receive the gift card.
